The Request for Information Access must contain the following:
- name and head office of the public authority body to which the request is submitted;
- data important for the recognition of requested information;
- name, surname and address of a natural person who is submitting the request, or the company i.e. the name of the legal entity and its head office.
A person/entity submitting the request does not have to state any reasons for requesting information access. Said request may contain the proposed manner for making information available by a public authority body.
